Bem-vindos queridos amigos e leitores deste blog! Hoje com um post mais pessoal e intimista e que pretende revelar um pouco do que é viver num país desorganizado e até terceiro mundista que é Portugal e, dar-vos a conhecer um pouco do que é a vida de um doente num país com estas características!

Welcome, dear friends and readers of this blog! Today's post is more personal and intimate and aims to reveal a little of what it's like to live in the disorganised and even third-world country of Portugal, and to give you a taste of what life is like for a patient in such a country!

Recentemente, uma reportagem de jornalismo de investigação revelou a dura realidade do financiamento dos medicamentos disponíveis para combater a Paramiloidose em Portugal - doença de que padeço - A doença apresenta-se inicialmente com sintomas leves que podem ser parcialmente controlados com o medicamento Tafamidis - comprimido diário - Este medicamento , é um estabilizador da proteína TTR que o fígado produz excessivamente. Ele custa 62 000€ por doente. No entanto, o Tafamidis não é uma cura definitiva e apenas retarda a progressão da doença. Eu tomei este medicamento durante 5 anos!

Recently, an investigative journalism report revealed the harsh reality of funding for the drugs available to combat Paramyloidosis in Portugal - a disease I suffer from - The disease initially presents with mild symptoms that can be partially controlled with the drug Tafamidis - a daily tablet - This drug is a stabiliser of the TTR protein that the liver produces excessively. It costs 62,000€ per patient. However, Tafamidis is not a definitive cure and only slows down the progression of the disease. I took this drug for 5 years!

Quando a doença se agrava, afectando orgãos vitais como o coração, rins, intestinos, membros inferiores, os doentes são transferidos para um segundo tratamento mais eficaz, um inibidor da proteína TTR, o Patisiran. Este tratamento custa 120 000€ por doente. A lógica financeira actual do sistema de saúde português prefere adiar este tratamento mais eficaz devido ao seu custo elevado, forçando os pacientes a passarem por um calvário de complicações antes de receberem o medicamento adequado.

When the disease worsens, affecting vital organs such as the heart, kidneys, intestines and lower limbs, patients are transferred to a second, more effective treatment, a TTR protein inhibitor, the Patisiran. This treatment costs €120,000 per patient. The current financial logic of the Portuguese health system prefers to postpone this more effective treatment due to its high cost, forcing patients to go through an ordeal of complications before receiving the right medicine.

A política de contenção de despesas acaba por ser um tiro pela culatra. Enquanto se economiza ao adiar o tratamento mais eficaz, o Estado acaba por gastar mais em intervenções secundárias, como implatanção de pace-makers, tratamento de insuficiência renal e tantas outras complicações. Além disso, a deterioração da saúde dos doentes muitas vezes resulta na perda da sua capacidade laboral, o que gera um impacto económico e social ainda maior.

The policy of containing expenditure ends up backfiring. While saving money by postponing the most effective treatment, the state ends up spending more on secondary interventions, such as the implantation of pacemakers, the treatment of kidney failure and many other complications. What's more, the deterioration in patients' health often results in them losing their ability to work, which has an even greater economic and social impact.

Uma luz no fim do túnel parece ser um novo medicamento administrado através de injecções trimestrais, evitando as longas sessões hospitalares de administração intravenosa a cada 3 semanas, que o segundo tratamento exige. No entanto, apenas sete pacientes em Portugal estão actualmente a receber este medicamento. O mais alarmante é a existência de uma lista de espera para o segundo tratamento, não por falta de medicamentos, mas devido à insuficiência de recursos nos hospitais, como cadeiras, salas e pessoal especializado!! A sério!!

A light at the end of the tunnel seems to be a new drug administered through quarterly injections, avoiding the long hospital sessions of intravenous administration every 3 weeks that the second treatment requires. However, only seven patients in Portugal are currently receiving this drug. The most alarming thing is that there is a waiting list for the second treatment, not because of a lack of medication, but because of insufficient resources in hospitals, such as chairs, rooms and specialised staff!!! Seriously!

Este contraste com outros países europeus, onde os doentes são tratados desde o início com os medicaments mais eficazes, é desolador. É triste ver que, enquanto os nossos governantes gastam recursos de maneira ineficente, muitas pessoas sofrem e são forçadas a emigrar para obter o tratamento e a valorização profissional que merecem!

This contrast with other European countries, where patients are treated from the outset with the most effective medicines, is heartbreaking. It's sad to see that, while our rulers spend resources inefficiently, many people suffer and are forced to emigrate to get the treatment and professional development they deserve!

Esta situação levanta uma questão crucial: por que o Estado Português não investe mais na saúde das pessoas, prevenindo a progressão da doença e evitando custos maiores no futuro? É imperativo que haja uma reavaliação das prioridades no financiamento da saúde, colocando a qualidade de vida dos doentes em primeiro lugar. É necessário um sistema de saúde que realmente responda aos problemas e necessidades dos seus cidadãos, garantindo um tratamento adequado e oportuno para todos. Mas ao que parece agora vamos ter que gastar mais dinheiro com a defesa! A guerra espreita! E, muitos oportunistas ganham milhões com a desorganizaação que grassa nos hospitais e suas administrações! Tenho dito!

This situation raises a crucial question: why isn't the Portuguese state investing more in people's health, preventing the progression of the disease and avoiding greater costs in the future? It is imperative that there is a reassessment of priorities in health funding, putting patients' quality of life first. We need a health system that really responds to the problems and needs of its citizens, guaranteeing appropriate and timely treatment for all. But it looks like we're now going to have to spend more money on defence! War is looming! And many opportunists are making millions from the disorganisation of hospitals and their administrations! I have said it!

The new generation seems to dispense with attending the National Conservatory or an acting training course. It's remarkable that nowadays the skill of dissimulation, the art of acting, has become commonplace for many people with all this happening in real life!

No entanto, enquanto alguns prosperam na arte da ilusão, é crucial reconhecer que a vida não é uma narrativa linear e previsível. O enredo pode tomar rumos inesperados e, é aqui que entra em cena a Lei do Retorno. Essa Lei diz-nos que as acções de uma pessoa, sejam elas boas ou más, eventualmente retornarão a ela, trazendo consigo as consequências das suas escolhas. Resumidamente há pessoas que actuam na vida real como se estivessem a viver uma boa novela ou uma série envolvente da Netflix!

However, while some thrive on the art of illusion, it is crucial to recognize that life is not a linear, predictable narrative. The plot can take unexpected turns, and this is where the Law of Return comes into play. This Law tells us that a person's actions, whether good or bad, will eventually return to them, bringing with them the consequences of their choices. In short, there are people who act in real life as if they were living a good soap opera or a gripping Netflix series!

No entanto, é crucial notar que, por trás da máscara da actuação diária, a vida reserva as suas próprias reviravoltas. A Lei do Retorno tece as suas consequências para aqueles que optam por enveredar pelos intrincados enredos da dissimulação. A vida, afinal, não é apenas uma encenação, as escolhas feitas hoje moldam o desenrolar das futuras cenas da vida real.

However, it is crucial to note that, behind the mask of daily performance, life has its own twists and turns. The Law of Return weaves its consequences for those who choose to engage in the intricate plots of dissimulation. Confesso-vos que ao ver assim as minhas fotos é mesmo numa heroína que me sinto, tantas têm sido as batalhas. Espero não vos decepcionar, mas isso são vocês que vão dizer! O que é certo é que o assunto da IA está na agenda mundial.

A IA, como em tudo nesta vida tem prós e contras, mas na minha opinião tudo se resume, à tentativa do Homem querer "brincar" de Deus! E, essa brincadeira pode ser muito perigosa. Segundo se diz Deus criou o Homem à sua imagem e semelhança. Eu acredito que nós para Deus somos a sua biotecnologia - já pensaram nisto nestes termos? - robots de conversação com sentimentos, vivemos num corpo que não nos pertence, mas sentimo-lo e, como sentimos!!É isso que estamos a tentar fazer com a IA - desenvolver biotecnologia pensante! Brincar de Deus!

AI, like everything else in this life has pros and cons, but in my opinion it all boils down to man's attempt to "play" God! And, that play can be very dangerous. It is said that God created man in his image and likeness. I believe that we are God's biotechnology - have you ever thought of it in these terms? - talking robots with feelings, we live in a body that does not belong to us, but we feel it, and how we feel!!!That is what we are trying to do with AI - develop thinking biotechnology! Playing God!

Há dias, li uma notícia que dava conta de vátios jovens terem afirmado que os seus smarphones ouviam as suas conversas. É a paranóia generalizada, é que isto é mesmo verdade! Os microfones instalados nos smarphones escutam as conversas e inundam-nos o smartphone com anúncios relacionados com essas conversas que tiveram. Sabem o que isto significa? Que os smartphones não só ouvem como fazem um raciocínio, ainda que simples (mas isto é só o começo de um novo mundo) de fazer associações com palavras! Quanto ao ChatGPT4 fica para um outro post, isso são outros quinhentos!!!

Tenho a certeza que conhecem a canção dos U2 "New Year's Day", é sobre o seu significado que vou falar neste dia de Ano Novo! Ouvindo a música podemos ser levados a pensar que se trata de uma música de amor, daquelas que se cantam neste dia especial do ano. Mas não! é uma música altamente politizada. Listening to the song we can be led to think that it's a love song of those that are sung on this special day of the year. But not! it's a highly politicized song. The Woke movement let's rest assured that the white that the letter refers to has nothing to do with the skin color, but with snow! The world in white that refers to the lyrics of the song are the countries of Eastern Europe!

Para nos situarmos no tempo, estamos nos anos 80, Thatcher governa no Reino Unido, políticas neoliberais, privatizações e uma nova ordem mundial desenha-se no horizonte culminando com a queda da União Soviética! Na cena mundial novos protagonistas começam a emergir! Em Portugal, o Governo formado por PS e PSD após as legislativas de Abril de 1983 recorreu à ajuda e intervenção do Fundo Monetário Internacional (1983-1985) e teve como objectivos principais ajudar o país a equilibrar as suas contas com o exterior, melhorar as contas públicas e as condições de crescimento económico.
